Project Completion Form

Please complete this form after your charger has been installed and is operational. In addition to this form, you will need to email* the following to ev@unitil.com:

Photos of the charger and its unit/station identifier.
Your electrician's final invoice marked "Paid in Full" showing:
- the installation address
- the installation Electrician's name
- MA Electricians License Number
- Electrician’s address and phone number
- the date the work was completed
- the work that was completed
- the cost of labor and materials.
